Poinsettia Order Form

Please make check payable to Sully First Reformed Church, write “Poinsettia” in the lower left corner of your check, and attach to Order Form. Please drop off payment in the church office. Please do not put your check and order form in the offering plate and do not include with your offering on one check; this causes confusion and will delay the plant order.
